For many of us, invertase is the more practical option
Sucraid may be the official treatment, but “official” does not help much when you cannot reliably get it. If you can’t get Sucraid covered, you’re not out of options. In fact, there’s an enzyme called invertase that functions through the same sucrose hydrolysis mechanism as Sucraid and is a better practical option than Sucraid in several key ways.
And practicality is not a side issue. Sucraid can be extremely difficult to live with, and access depends on multiple factors we have little to no control over, such as insurance policies and cost. For a condition like sucrase deficiency, practicality and convenience matter so much because eating is something we do multiple times every day. This loss of control is a huge part of why Sucraid can be so frustrating. That’s why an option like invertase is important: it gives us hope for a treatment that we can actually use on our own terms.
So, here are ways in which invertase is a better option:
- Invertase is much cheaper. In one study, the estimated annual cost of invertase and Sucraid was compared, and invertase was estimated to cost £1,972 (roughly converted to $2,650) per year and Sucraid was estimated at £51,938 (roughly converted to $70,000) per year (Morris et al.).
- Invertase is much more accessible. In one insurer’s coverage policy, Sucraid requires a confirmed sucrase deficiency as part of a CSID diagnosis via small bowel biopsy with disaccharidase assay or a qualifying genetic test. Patients also need prior authorization submitted by their prescriber (Blue Cross Blue Shield of Michigan / Blue Care Network). Not every insurer requires these prerequisites specifically, but requirements like a confirmed diagnosis and prior authorization are some of the most common access barriers. After jumping through those hoops, your insurance has to agree to cover it at a massive price. Morris et al. estimated Sucraid at roughly $70,000 per year, and current U.S. price estimates can put it north of $100,000 per year depending on dosing and pricing source. For example, Drugs.com lists 300 mL of Sucraid at more than $12,000, while the FDA prescribing information recommends 2 mL with each meal or snack for patients over 15 kg (Morris et al.; Drugs.com; FDA – Sucraid Prescribing Information). The math on that comes out to about $120,000 per year if you take the recommended 2 mL dose with three meals and one snack each day. There are also personal accounts of coverage being unexpectedly disrupted or outright denied after it was previously approved. Meanwhile, invertase can simply be bought as an over-the-counter supplement without any dependence on specialty pharmacies and insurance coverage.
- Invertase can be bought in powder or capsule form. That matters because the form makes it more practical. With powder, you have more flexibility in how much you use in a given sitting instead of being locked into Sucraid’s liquid medication format that is harder to store, carry, and adjust relative to the amount you’re eating. You can also mix invertase powder directly into food or liquid, which may start breaking down sucrose before you eat it, depending on timing and the food. That is essentially how invertase has been used in confectionery and candy-making for about a century (NCBE / University of Reading). The most convenient version for everyday life is probably invertase in powder form stored in capsules. Brands like Starchway sell capsules containing invertase (Intoleran) and capsules are much easier to keep with you, travel with, and use discreetly than refrigerated liquid Sucraid. Pure invertase powder stored in bulk is different and may not be as convenient, especially for travel. For example, this invertase product from Amaris (Amaris Chemical Solutions) is to be stored at 2–8°C (35.6-46.4°F) in a dry, airtight container. However, capsule-based invertase is much more practical for the way people actually live and eat.
- Invertase can be very useful in emergencies. If you are someone that typically has access to Sucraid but something goes wrong, invertase is easier to obtain and keep available, especially in capsule form, making it a good backup option to keep on hand just in case.
Overall, FDA-approved Sucraid is not very practical for a lot of us, and the convenient alternative, invertase, has not been clinically supported in the same way. Sucraid has clearer regulatory and clinical validation because its FDA prescribing information identifies it as an oral enzyme replacement therapy for sucrase deficiency as part of CSID (FDA – Sucraid Prescribing Information), making it the only option discussed here with that level of regulatory and clinical credibility. However, invertase has major practical advantages, making it the better practical option for many of us in various ways.
